STEP 1: Download the IRS W9 Request for Taxpayer ID form. Bring page 1 completed and signed to Art Intake. This is required in order to pay you any commissions on art sales.
STEP 2: Complete the Art Intake Inventory form a minimum of two weeks prior to your designated art intake date. You will be required to sign a Works on Consignment Agreement form when you bring your artwork to the Center. Additional rows may be added to the form to accommodate the number of works presented.
